Follow these simple Steps in order to make your introduction to our office as simple as possible:

1. Fill out our online New Client Form:  Click Here to save a tree or PRINT the form and bring it in with you.
2. Download an Insurance Authorization Form Here to bring in with you: INS Form Here
3. Feel free to browse through some of the Federal Government rules and regulations regarding Patient privacy (HIPPA).  We will have forms for you in our office to sign acknowledging your HIPPA understanding your acknowledgement to allow us to ring you (Contact form) and our “open” adjusting setup.
4. Prepare yourself to reach your goals and achieve a state of wellness that you may have only dreamed about.  Set a standard for yourself that is not only much higher than you are used to but a standard of living that serves you as a human.
